This form is example of multiple page forms that can be used for questionnaires or wizards. Similar to Feedback form allows users to rate our site and send feedback. By clicking on Next button new page appears depending on the user choice in first step.

If user selects rating 1 or 2 in first step, textbox phone will be displayed in next step. If user selects 3, 4 or 5 in first step, checkbox to receive newsletters will be displayed on next page. Data are saved by clicking on "Finish button" button.

This example uses only two steps for wizard (and very simple forms, only one field) but you can use as many steps as you want (with as many fields on each page as you want). You can build questionnaires using tree (first step 1 page, second step 2 pages, third step 4 pages) or skip pages or show some pages only on specific date or for specific users.

For purposes of this example we used same table Comments as for Feedback form example. We defined mail as required field (mail syntax is also checked - standard and custom client and server side validators are implemented in grid) and allow user to choose from predefined list of rates.
