This form allows users to rate our site and send feedback. Data are saved by clicking on "Submit" button.

For purposes of this example we have defined new table Comments to store data (using IndooGrid table wizard). We defined mail as required field (mail syntax is also checked - standard and custom client and server side validators are implemented in grid) and allow user to choose from predefined list of rates. After users clicks Submit button he is redirected to Thank you page.
